The deadline to finish the LOA APPLICATION process is on the 7th day of May 2026 (Thursday).
It is expected that all steps have been done on or before the deadline. These include getting your instructor's consent, having your adviser "note" your application, having your application endorsed by your dept/college (if applicable), approved by your dean, and paying the fees indicated in the dropping/payment slip.
Students who will be applying for LOA are reminded that it is your responsibility to ensure that the process is finished on or before the deadline.
Submit Request: Go to the "Leave of Absence" module on CRS and submit your reason for LOA.
Instructor's Consent: Inform your instructor and ensure they submit their consent via CRS..
Adviser Approval: Inform your Program Adviser to approve the request on CRS.
Department Endorsement: The Department Chair/Program Director will endorse the your application.
College Approval and Print LOA Form: The College Secretary will review and approve your application.
Dean’s Approval: The Dean will review and approve your application.
Payment at UPD Cash Office: Go to the Office of the College Secretary to request for the print out of the LOA Form and pay P150 to the UPD Cash Office.
Payment Online: Generate a payment slip, pay the dropping fee via the "Settlement of Outstanding Transaction" module in CRS (e.g., GCash, LinkBiz), and have it validated.
Return from LOA Form: Download and fill out the form (here).

Submit to OCS: Submit to the Office of the College Secretary the physical/hard copies of the documents. Print the documents in A4 sized bond paper or email to:
SUBJECT: [RETURN FROM LOA] LASTNAME, FIRSTNAME
Once your submission has been received, the Office of the College Secretary (OCS) will forward your application to the Office of the University Registrar (OUR).
The OUR and the Computerized Registration System (CRS) will update your CRS account once your application is approved.
